Diabetes is a disease that is viewed as a group of metabolic conditions. Diabetes, sometimes referred to as Diabetes Mellitus, has four different stage classifications.

These stages include the following designations;• Type 1 Diabetes• Type 2 Diabetes• Gestational Diabetes• Pre-Diabetic

Now let’s take a closer look at each stage of this disease

Type 1 Diabetes

Type 1 Diabetes also known by the names insulin-dependent diabetes, ketosis prone diabetes, childhood diabetes, and juvenile diabetes. It should be noted that some of these terms may be outdated. Right now in the US, type 1 diabetes is diagnosed in 5 to 10 % of Americans.

Type 1 diabetes is characterized as the body’s inability to produce insulin. When the body does not produce natural insulin this happens because the pancreas, which has what are called beta cells are being destroyed and as this happens, insulin is no longer being produced.Diagnosis of type 1 diabetes is a disease that can be diagnosed throughout someone’s life time, but it usually is diagnosed in young adults and in most often in children.

While diabetes knows no boundaries as far as who can have the disease, type 1 diabetes is diagnosed usually more in Caucasians than any other segment of the population including African Americans or Latinos. It should also be noted that the disease is equally spread out between females and males.

Type 2 Diabetes

Type 2 diabetes is characterized as insulin resistance. This resistance combined with the body’s inability to secrete insulin causes our body’s tissues to be less responsive to insulin and is believed to involve our insulin receptors.

Type 2 diabetes is prevalent in adults whose ages range from age 50 and older, but it can sometimes be diagnosed in young children. A main cause of the disease is due to obesity and or just being overweight and not doing enough or any type of exercise. Leading a sedentary lifestyle can be very detrimental to a person who has type 2 diabetes.

Another factor that is directly related to the lifestyle of the type 2 diabetic is high blood pressure glucose levels. High blood pressure glucose levels, can cause issues with every organ in the body over time and may eventually cause high blood pressure, kidney failure and nerve damage. Fortunately, type 2 diabetes can be controlled through life style adjustments such as exercise, proper medications and diet.

Gestational Diabetes

Diabetes that occurs in pregnancy is referred to as Gestational Diabetes, it can resolve at birth. There are several risk factors that can account for this occurrence which include the following…

• Being pre-diabetic

• Having given birth previously to a child weighing 9 pounds or more

• Being overweight

• Diabetes runs in your family.This form of the disease seems to affect just about all classes of people including Asian Americans, Latino Americans, and African Americans.

At an estimated 5 % or higher of American women who have this particular type of diabetes that translates to around 200,000 American women or more. The main reason for Gestational Diabetes is due to women not being able to metabolize glucose properly. In this case the body is able to produce or secrete the proper amounts of insulin, but unfortunately due to the condition known as insulin resistance, the body does not allow the person suffering from this disease to utilize the insulin properly.

Pre-Diabetic

What exactly does it mean to be someone who is a pre-diabetic? Pre-diabetes occurs when a person’s blood sugar ( glucose ) levels are above what are considered to be normal levels, but these levels are not considered to be high enough to be diagnosed as actually having a full blown diabetic condition. Tests that are performed in order to diagnose this condition are a Fasting Plasma Glucose test or FPR for short, or an OGTT which is an Oral Glucose Tolerance test.Both of these tests involve the person to fast overnight and then have the test conducted early in the morning, before a meal is consumed.

The OGTT test is performed in two stages; the first test is administered after the person has fasted and then again two hours later, after glucose rich drink is consumed by the patient.Knowing if you’re pre-diabetic gives the person diagnosed positively with the chance to defend against becoming diabetic by changing their lifestyle habits. Fat is also a problem with increase incidences of atherosclerosis, large vessel disease and cardiac complications. What, then is the appropriate macronutrient for the diabetic population? Enough medical literature exists to suggest that in diabetes, proteins are probably the best bet.

Proteins are the natural choice of the body when faced with diabetes. In uncontrolled diabetes, muscle protein is broken down into amino acids to be converted into glucose by the liver. If left to fend for itself, this can create a commotion within the body. Since proteins have to supply enough energy to substitute for carbohydrates, proteins are broken down faster than they are made. The body ends up with a protein deficit, a situation with subtle, yet far-reaching effects on normal body functions. Importantly, for diabetics, a protein deficit has been shown to impair resistance to infections (Ganong WF). Replenishing the depleting protein stores is a vital requirement of all diabetic diets.

Importance of proteins in a diabetic has been well documented. The American Associations of Clinical Endocrinologists have made it clear that not much evidence exists to indicate that the patients with diabetes need to reduce their intake of dietary proteins. The AACE recommends that 10-20% of the calorie intake in diabetes should come from proteins (AACE Diabetes Guidelines). It is in fact believed that this is one nutrient that does not increase blood glucose levels in both diabetics and healthy subjects (Gannon et al).

Nutrition therapy for diabetes has progressed from prevention of obesity or weight gain to improving insulin’s effectiveness and contributing to improved metabolic control (Franz MJ). In this new role, a high protein diet (30% of total food energy) forms a very pertinent part of nutrition therapy. One of the most important causes for type II diabetes is obesity. Excess body fat raises insulin resistance and higher levels of insulin are required to bring down blood sugars as the weight increases (Ganong WF). Another problem with excess fat is the clogging of arteries with atherosclerotic plaques that is responsible for a wide range of diabetic complications. Any mechanism that reduces body fat, decreases insulin resistance, and improves blood glucose control. Parker et al have also shown that a high protein diet decreased abdominal and total fat mass in women with type II diabetes. Other studies by Gannon et al. and Nuttall et al have verified that blood glucose levels and glycosylated hemoglobin (a marker of long term diabetic control) reduce after 5 weeks on a diet containing 30% of the total food energy in the form of proteins and low carbohydrate content. It is speculated that a high protein diet has a favorable effect in diabetes due to the ability of proteins and amino acids to stimulate insulin release from the pancreas. Thus, a high protein diet is not only safe in diabetes, but can also be therapeutic, resulting in improved glycemic control, and decreased risk of complications related to diabetes.

The benefits of a high protein diet do not end here. Individual protein components of such a diet, when aptly chosen, can have other advantages as well. Dietary supplements containing proteins like whey and casein come highly recommended. Casein is a milk protein and has the ability to form a gel or clot in the stomach. The ability to form this clot makes it very efficient in nutrient supply. The clot is able to provide a sustained, slow release of amino acids into the blood stream, sometimes lasting for several hours (Boirie et al. 1997). A slow sustained release of nutrients matches well with the limited amount of insulin that can be produced by the pancreas in diabetes. A protein supplement containing casein can thus increase the amount of energy assimilated from every meal and, at the same time, reduce the need for pharmacological interventions to control blood sugar.

Whey proteins and caseins also contain “casokinins” and “lactokinins’, (FitzGerald) which have been found to decrease both systolic and diastolic blood pressure in hypertensive humans (Seppo). In addition, whey protein forms bioactive amine in the gut that promotes immunity. Whey protein contains an ample supply of the amino acid cysteine. Cysteine appears to enhance glutathione levels, which has been shown to have strong antioxidant properties — antioxidants mop up free radicals that induce cell death and play a role in aging.

Thus, development of a protein supplement containing casein and whey can provide an apt high protein diet and its health benefits to individuals suffering from diabetes, obesity, and hypercholesterolemia.

Diabetic complications can and almost certainly WILL arise when you do not look after your condition properly, be it type1 or type 2 diabetes. Diabetic complications mainly arise when you do not monitor your blood glucose levels or let them run continuously high. Eyes – What happens to my eyes and diabetes? Diabetics risk the eye complication called diabetic retinopathy. Diabetic retinopathy affects the tiny blood vessels that supply your retina and can become leaky, blocked and grow abnormally. You need to avoid diabetic retinopathy as your sight is valuable and with diabetic retinopathy, you could lose your sight.

Keeping your blood glucose levels under control can reduce the risk, although blood pressure and blood fat levels will also affect this.

Each year it is advised to get your eyes screened. This is done by visiting your health centre for a short eye test, this is different from the usual optometrist test, as the screening involves having pictures taken with a digital camera to view the backs of your eyes and the consultant can see the small vessels and look for any problems such as diabetic retinopathy. Many opticians do this when dealing with their diabetic customers and is included with your eye test.

You may need to go and see the specialist at the hospital for them to investigate further. If diabetic retinopathy is diagnosed and bleeding is out of control, you may need laser treatment which will prevent any growth of abnormal blood vessels. Diabetic retinopathy laser treatment is different from the one that are widely advertised on T.V, as it cannot restore any vision loss.

Ears – It has been found that hearing loss affects more people who have diabetes than their counterparts. High blood glucose can affect the small vessels and nerves in the inner ear and can affect our hearing over time.

The Kidneys – Diabetes can affect the kidneys by damaging the small blood vessels which can become leaky or even stop working, this is called diabetic nephropathy. By keeping blood glucose levels at 4mmol/l or 72mg/dL and 6mmol/l or 108mg/dL before meals and 10mmol/I or 180mg/dL 2 hours after food can greatly reduce risks of kidney disease such as this diabetic nephropathy.You will be asked to bring a sample of urine (the first that you pass in the morning) to your annual check up. This is to test the urine for protein called microalbuminuria. If you have signs of microalbuminuria in the sample, the doctor is likely to prescribe medication called an ACE inhibitor in tablet form. This medication will help to prevent nephropathy from getting worse. If untreated, this could lead to kidney failure.

Feet/Nerves – The nerves carry messages from our brain to every part of our body, so we see, hear, smell and touch etc.

Diabetic neuropathy – A long term complication of diabetes is diabetic neuropathy. Diabetic neuropathy is nerve damage that results from diabetic micro vascular disease and is increased if you have poor diabetes control. It particularly affects the feet and legs. You may feel numbness, pain or feelings of an altered sensation. This is why you should have your feet checked regularly by your chiropodist or Podiatrists. This type is known as ‘Sensory neuropathy’. Autonomic neuropathy – This affects nerves that control our automatic activities, such as actions by the stomach, intestines, bladder and penis (Erectile dysfunction), and even the heart.

Hypoglycaemia

Hypoglycaemia or the ‘hypo’ is the most common of diabetic complications that a diabetic will come across at some time. Basically, hypoglycaemia appears when our blood glucose levels go just below 4mmol/l or 72 mg/dL, although this really depends on how controlled your blood glucose levels are. When my sugars have been out of whack, I have felt ‘hypo’ at around 6mmol/l or 108 mg/dl, Really! which is a good reason to test your blood with a meter if possible when you feel a little ‘odd’ to make sure that you really are ‘hypo’ but if you cannot, just treat as a ‘hypo’, as this needs to be treated as soon as possible. If left, you could become unconscious and will require a Clucagon injection to bring you round. If left untreated, you could risk having a diabetic coma. Diabetes is well known metabolism disorder where the body is unable to manage the sugar level in the body. There are two types of diabetes, known as type 1 and type 2 Diabetes. Type 1 diabetes is called insulin dependent diabetes because the body cannot produce insulin. Type 2 diabetes is called insulin resistant diabetes because the body cannot control the insulin level. There are many misconceptions about the disease which will be discussed in detail in this article.

The first myth is that people who are overweight will get diabetes sooner or later. There is some truth in it since obese people are predisposed to Type 2 Diabetes. To say that an overweight person will be diabetic is not accurate at all. There have to be other factors playing for diabetes to be acquired as there are many people who are overweight and with very healthy blood sugar levels.

Diabetes is not contagious. Simply having a family member with it or interacting with diabetics for a long time will not ‘transmit’ the condition to you. Diabetes can be inherited though, if a parent or a grandparent has it. The likelihood of this happening is not high all the time, especially if your parents are free of diabetes.

But diabetes is a manageable disease and as long as you keep your diet in check, exercise regularly and keep a happy lifestyle, then it will never be a problem for you.
